Health Leads at
 West County Health Center


Through our partnership with Health Leads USA, we connect patients at West County Health Center with needed services and resources addressing underlying socioeconomic issues that affect health.

This partnership, which is supported by a grant from the Kaiser Community Benefit Fund, allows our doctors to "prescribe" services to address issues such as hunger, lack of adequate housing and unemployment. Volunteers at the onsite Health Leads desk help patients find services such as:

  • Health insurance
  • Food pantries and food assistance programs
  • Gas and electric turn-off assistance and discounts
  • Job training
  • Daycare, child care and afterschool programs
  • GED, English classes and college programs
