Alternative Access Standards for Specialty Providers


Welcome to Contra Costa Health Plan's Alternative Access Page


The time and distance standard states that a Primary Care Provider (PCP) must be within 10 miles and 30 minutes of a member’s address and that a specialty provider must be within 15 miles and 30 minutes of a member’s address. Per Welfare and Institutions Code Section 14197.04(c), Contra Costa Health Plan (CCHP) requested Alternative Access for these zip codes to the nearest out of network provider. CCHP is making every effort to contract with a provider within the alternative access requested.

If your zip code and provider type are listed below, Contra Costa Health Plan (CCHP) must help you find an appointment. You have a right to call Member Services department at 1-877-661-6230 to get help with finding an appointment to a closer specialist. CCHP will then issue a single patient agreement for medically necessary services. If CCHP cannot find you an appointment with a closer specialist and you need assistance with transportation, CCHP will arrange transportation to see the specialist.
